It’s not actually your brain freezing — nothing in your skull gets cold enough for that — but the real explanation is still a fascinating case of the body overreacting to a cold surprise.

The Leading Explanation: A Blood Vessel Response

According to Harvard Health, eating or drinking something very cold causes blood vessels in the palate — the roof of the mouth — to rapidly constrict as a survival reflex meant to protect the body’s core temperature, then just as rapidly reopen. The Cleveland Clinic describes a related mechanism: when the body senses sudden extreme cold in the mouth or throat, blood vessels throughout the head expand to let extra warm blood into the area. That fast swing in blood vessel size is what triggers the sudden jolt of pain.

Why the Pain Shows Up in Your Forehead

The mouth is nowhere near the forehead, so why does it hurt there? The trigeminal nerve is the likely link. Harvard Health notes that this nerve, which extends into the midface and forehead, carries the pain signal from the rapid vascular changes in the palate up to where it’s actually felt. It’s a case of referred pain — the same principle behind a heart attack sometimes being felt in the arm — rather than any cold actually reaching brain tissue.

Anyone can get brain freeze, but the Cleveland Clinic notes kids tend to get it more often, largely because they’re more likely to eat cold treats quickly rather than pace themselves. Research also points to a higher prevalence among people who are prone to migraines, suggesting some shared sensitivity in how their blood vessels and nerves respond to rapid temperature change. Notably, Harvard Health is candid that the precise cause of cold-stimulus headaches “remains a mystery” — the vasodilation theory is the leading explanation, not an airtight, fully proven one.

How to Stop One in Its Tracks

If a brain freeze strikes, the Cleveland Clinic recommends three simple fixes: stop eating or drinking the cold item, sip something warm or at room temperature, or press your tongue or thumb against the roof of the mouth to transfer warmth to the area. To avoid it altogether, eating very cold foods slowly — giving the mouth time to adjust — is the most reliable prevention.
