Unlike most diets, it doesn’t focus on what you eat or how many calories you count — it’s entirely about timing, and researchers are still working out exactly how much that timing matters.

Johns Hopkins Medicine defines intermittent fasting as an eating pattern that limits food intake to specific windows of time, rather than restricting particular foods or food groups. The idea is to support a metabolic shift: after enough hours without food, the body exhausts its readily available sugar stores and starts burning stored fat for fuel instead. In a culture of near-constant snacking, proponents argue, the body rarely gets the extended gap it needs to make that switch.
The Most Common Schedules
Several patterns dominate the research and popular practice. The 16:8 method restricts eating to an eight-hour window each day, with 16 hours of fasting in between — often achieved simply by skipping breakfast or dinner. The 5:2 approach involves eating normally five days a week and limiting intake to about 500 to 600 calories on two non-consecutive days. Alternate-day fasting, a third pattern studied by the National Institute on Aging (NIA), cycles between normal eating days and days of significant calorie restriction.
What’s Actually Happening Metabolically
Researchers writing in the New England Journal of Medicine, summarized by the NIA, describe the core mechanism as “metabolic switching”: during a fasting period, the body shifts from burning glucose to burning fat, producing ketone bodies in the process. Those ketones aren’t just a backup fuel source — the researchers note they also act as signaling molecules that can trigger cellular responses, including better defense against oxidative stress, improved glucose regulation, and reduced inflammation.
What the Evidence Does and Doesn’t Show
The NIA review found that intermittent fasting has shown benefits related to obesity, diabetes, cardiovascular disease, and some neurological conditions. But the authors are careful to note real limits: evidence for effects on human lifespan is much less clear, animal studies have produced mixed results depending on sex and diet composition, and most human trials have been short-term and focused on middle-aged adults who were overweight. Data on healthy-weight people, and on younger or older populations, remains limited.
Who Should Be Cautious
Intermittent fasting isn’t appropriate for everyone. Johns Hopkins Medicine advises against it for children and adolescents under 18, pregnant or breastfeeding women, people with type 1 diabetes who use insulin, and anyone with a history of eating disorders, since strict eating windows can trigger disordered eating patterns. As with any significant change in eating habits, medical guidance is recommended before starting, particularly for anyone managing an existing health condition.
